Insurance Information
Full payment is expected at time of service, regardless of insurance coverage. If treatment is covered by insurance, a claim will be submittd on your behalf. MCFPS participates only with Medicare. Therefore, payment on claims for all other insurance companies will be sent directly to the subscriber. In these cases, full payment from patient is required. In case of medicare coverage, applicable co-payment will be collected at time of service.
